PRIVACY & COOKIES

Your question stays with the directory team.

The contact form is for directory questions and corrections. Please do not include patient information, medical records, identity documents or other sensitive health data.

What we collect and why

The form collects your name, email address, question, submission time and an optional reference to a clinic record. These are stored in the application database so the administrator can read and respond to directory questions and investigate corrections. Submitting the form is optional. The checkbox records your acknowledgement of this notice; it is not consent to marketing.

Your message appears only in the protected administrator dashboard. It is not published, emailed automatically, or forwarded to a clinic. An email address is requested so the administrator can contact you if needed; this version has no automated email delivery or visitor inbox. Do not rely on this form for urgent or medical assistance.

Storage, recipients and retention

Questions are stored in Cloudflare D1 with the database restricted to the European Union. Cloudflare operates the hosting, database and anti-abuse services. The source collector in Türkiye publishes public clinic records; questions do not depend on that collector being online. Authorized administrators and the hosting, database and backup operators may process them to operate the service. Ordinary question records are removed from the active database after 90 days by a scheduled Cloudflare task. Security attempt records use a keyed hash of the client address and are retained for up to two days to limit spam and login abuse. Database backups can retain older copies until the operator's backup rotation removes them.

Questions can contain personal data. Do not include patient information or medical documents. The operator handles privacy requests through this form. Cloudflare’s infrastructure and any backup copies require their own retention and international-transfer assessment.

Necessary session cookie

The administrator’s first-party sign-in cookie is HTTP-only, Secure, uses SameSite=Strict and expires after eight hours. A short-lived login cookie prevents forged sign-in requests. Cloudflare Turnstile checks form and login requests for automated abuse and processes browser and network signals. See Cloudflare’s privacy policy. No advertising or analytics cookies are installed by this application.

Your information and external links

Use the question form to request access, correction or deletion, identifying the address used for the original submission. The administrator must verify the request before disclosing personal information. See the Personal Data Protection Authority's explanation ↗. Official source links open separate sites with their own privacy practices. Operational request logs and backups require their own retention controls on the hosting server.
